Continuity of Care Clinic

Our continuity of care clinic, located in the Medical Arts Pavillion at SCH, is the hub of the family medicine residency. First year interns will be scheduled one ½ day session per week to begin to build their patient panel and second and third year residents are schedule at the clinic two days per week. In the Continuity of Care Clinic under the supervision of their clinic preceptor, residents will develop an ambulatory patient panel of their own. These patients will view the resident as their primary care provider, and the resident will direct and manage their care for the duration of their time in the clinic. These longitudinal relationships with patients can be the most rewarding aspect of Family Medicine training.

We highly value the Continuity Clinic experience for residents and their patients; it both challenges and rewards each resident through a complex array of medical issues and a culturally diverse patient population. Many of our patients are Hispanic/Latino with Spanish being their primary language; use of an interpreter service is available when a resident confronts a language barrier. Culturally appropriate care exists as a core feature of learning within our office. We have access to a superb group of subspecialists who provide consultation, as well as resident teaching, within internal medicine, pediatrics, adult health and its many divisions, as well as within the realm of mental health services. We strive to continuously offer high-quality, evidence-based care that is patient-centric. We are committed to care of the disadvantaged, and to training future generations of family physicians. We firmly believe this is an ideal training environment for a 21st century family physician.
