Community Health Needs Assessment 2023
Suburban Community Hospital is dedicated to strengthening the health of our communities and creating a holistic healing environment where compassionate care and quality healthcare intersect. Suburban Community Hospital has a long history of serving the entire community, with a special concern for the disadvantaged and most vulnerable.
In the past, Suburban Community Hospital has employed different methods to assess and address the health needs of the communities it serves within the Philadelphia Region. In 2012, Suburban Community Hospital collaborated with 28 hospitals through the Delaware Valley Healthcare Council (DVHC) to contract with the Public Health Management Corporation (PHMC), a private non-profit public health institute, to assist with data collection, research and initial prioritization of the health needs in the individual communities that are served by our hospital. The purpose was to gain a distinct understanding of the most pressing health issues affecting our communities, as well as develop an implementation strategy to improve the overall health of our communities. As described in each document below, the community’s unmet priority health care needs were identified based on the analysis of the data and community input, and then prioritized based on the needs each hospital could positively impact.
